a.) Proof Of Income: ITR, Employment Cetificate, Payslip, DTI, Business Permit, 6 Months Bank Statement
b.) Two (2) Valid ID's with signature (photocopy then 3 specimen signature) plus 2pcs 2x2 picture
c.) Proof Of Billing Barangay cetificate
d.) Approval Boosters Club membership payments, credit card billing, top 7,000 Corp ID
e.) Filled-out Application Form
f.) Maybe Asked: Spouse and/or co-maker ID and income details .

4. Wait for claim advise. Once approved, claim mo ang motor mo sa outlet nearest you. Nationwide!

1. Latest Pay slip
2. 2 pcs. 2x2 picture
3. Valid IDs – Two of the following maybe submitted:
